COVID-19 Vacination Confirmation Portal
OMHA has mandated that all eligible participants involved in minor hockey are to be fully vaccinated for COVID-19 by October 31, 2021.

All players born in 2009 or earlier, all team staff and on ice volunteers must be double vaccinated plus 14 days in order to be on an approved roster and must provide GMHA with such proof. 

To help with this we have a created a portal in which you can directly upload your vaccine confirmation, this needs to be completed by October 11th in order that we can have the approved rosters in place by month end.

Parents or companions are not part of this process as their status is not governed by the OMHA but by the policies of Public Health and the Town of Gananoque.
